When you child starts school there is some information that we require to ensure that we have any medical details about your child and emergency contacts. Please can you complete this form and return it to the school office to ensure our records maintain up to date.
Medicines Form
If your child requires medication during the school day you will need to fill in the 'Consent to give Medications' form below. Before completing the consent form pease make yourself aware of the second document in this section - 'Medicines -administering to pupils.' Both of these forms have recently been updated. Thank you.
Leave of Absence Request Form
Please read the governors guidance on accepting absences during school time before completing the form.
Combined permissions form - Photography, free fruit, trips around village, communication
In order for us to include your child in any photos that may be taken throughout the school day, we require your permission. We now have a combined ' Permissions Form' which incorporates a number of items that you are able to give your permission to or not as you feel fit.
